  4. ‡ LDL is calculated from measured values of total cholesterol, triglycerides, and HDL according to the Friedewald calculation for US NHANES 2001-2002: [LDL] = [total cholesterol] - [HDL] - [triglycerides/5] where all values are expressed in mg/dL. The calculation is valid for triglycerides less than or equal to 400 mg/dL.
  5. ¥ Current smokers were considered to subjects who have a life-time smoking experience of ≥ 100 cigarettes and currently smoke.
