  1. Analysis was performed to estimate hazard ratios (HR) with corresponding 95% confidence intervals (95% CI) for coronary heart disease associated with maximal body mass index before enrollment (MAXBMI).
  2. Reference categories were subjects with a MAXBMI between 22.0 and 24.9.
  3. Model 1 is adjusted for age and sex; model 2, model 1 plus smoking status, systolic blood pressure, total cholesterol and HDL cholesterol; model 3, model 2 plus HbA1c values at enrollment.
